Summary

Quinidine potently inhibits CYP2D6 and markedly raises dextromethorphan concentrations. This is used therapeutically in the fixed combination for pseudobulbar affect, but at higher exposures it may cause dizziness, sedation, confusion and, rarely, serotonin toxicity.

What this interaction means in plain words

Dextromethorphan and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dextromethorphan belongs to Respiratory; Quinidine belongs to Anti Parasitic.

Evidence level: High

Why it happens

Quinidine is a potent CYP2D6 inhibitor that blocks first-pass and systemic metabolism of dextromethorphan.

What to do

Avoid unsupervised use of additional dextromethorphan with quinidine; if combined, use low doses and monitor for CNS adverse effects and QTc prolongation.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
