Summary

Sparsentan, an endothelin and angiotensin receptor antagonist, reduces glomerular pressure, and NSAIDs can worsen kidney function in this setting. Sparsentan is also a weak CYP2C9 inducer, which may modestly reduce diclofenac exposure.

What this interaction means in plain words

Diclofenac and Sparsentan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diclofenac belongs to Musculoskeletal; Sparsentan belongs to Genitourinary.

Evidence level: Moderate

Why it happens

NSAID inhibition of renal prostaglandins combined with RAAS and endothelin blockade, plus CYP2C9 induction by sparsentan.

What to do

Monitor serum creatinine, potassium and blood pressure when an NSAID is used, and limit NSAID duration. Check diclofenac efficacy and consider an alternative analgesic.

What to ask your pharmacist
