Summary

Sparsentan, an endothelin and angiotensin receptor antagonist, lowers glomerular pressure, and NSAIDs reduce prostaglandin-mediated renal perfusion. Together they may worsen kidney function, particularly in volume-depleted or elderly patients, and can increase hyperkalemia risk.

What this interaction means in plain words

Diflunisal and Sparsentan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diflunisal belongs to Musculoskeletal; Sparsentan belongs to Genitourinary.

Evidence level: Moderate

Why it happens

Combined reduction in efferent arteriolar tone from receptor blockade and loss of afferent vasodilation from NSAID prostaglandin inhibition.

What to do

Avoid regular NSAID use where possible and monitor serum creatinine and potassium. Ensure the patient is adequately hydrated.
