Summary

Thiazide-induced hypokalemia and hypomagnesemia sensitize the heart to digoxin and raise the risk of digoxin toxicity and arrhythmias. Olmesartan may raise potassium and partly offset this, making the net effect variable.

What this interaction means in plain words

Digoxin and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Diuretic-induced electrolyte depletion enhances digoxin binding to Na+/K+-ATPase and myocardial sensitivity.

What to do

Monitor serum potassium and magnesium, and digoxin levels and symptoms of toxicity. Correct electrolyte abnormalities promptly.
