Summary
Thiazide diuretics such as methyclothiazide cause potassium and magnesium loss, and the resulting hypokalemia sensitizes the myocardium to digoxin and can precipitate arrhythmias. This is a clinically well-recognized interaction.
What this interaction means in plain words
Digoxin and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Diuretic-induced hypokalemia and hypomagnesemia enhance digoxin binding to Na+/K+-ATPase and its toxicity.
What to do
Monitor serum potassium and magnesium regularly and replace deficits, targeting normal potassium. Monitor digoxin levels and for toxicity symptoms.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
