Summary
Beta-2 agonists such as pirbuterol can lower serum potassium and may provoke tachyarrhythmias. In a patient taking digoxin, hypokalemia increases myocardial sensitivity to the glycoside and raises the risk of digoxin-related arrhythmias.
What this interaction means in plain words
Digoxin and Pirbuterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Beta-2 agonist-induced intracellular potassium shift causes hypokalemia, which potentiates digoxin cardiac toxicity; additive proarrhythmic effects may also contribute.
What to do
Monitor serum potassium and ECG when pirbuterol is used regularly or at high doses with digoxin, and correct hypokalemia promptly. Check digoxin levels if toxicity symptoms appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
