Summary
Both drugs cause sedation, so combined use can produce marked drowsiness and impaired psychomotor performance. Diphenhydramine's anticholinergic activity adds to that of risperidone and may cause confusion, urinary retention or constipation. Diphenhydramine also modestly inhibits CYP2D6, which may raise risperidone exposure.
What this interaction means in plain words
Diphenhydramine and Risperidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Diphenhydramine belongs to Respiratory; Risperidone belongs to Central Nervous System.
Why it happens
Additive CNS depression and anticholinergic effects, with possible CYP2D6 inhibition by diphenhydramine increasing risperidone levels.
What to do
Monitor for oversedation, anticholinergic effects and extrapyramidal symptoms, particularly in older adults. Prefer a non-sedating antihistamine when feasible.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
