Summary
Dolasetron's active metabolite prolongs the PR, QRS and QT intervals, and erythromycin is a recognized QT-prolonging macrolide. Combined use raises the risk of torsades de pointes, especially with electrolyte abnormalities or structural heart disease.
What this interaction means in plain words
Dolasetron and Erythromycin stearate can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive prolongation of cardiac repolarization (QT interval).
What to do
Avoid the combination where possible and choose a non-QT-prolonging antiemetic or antibiotic. If unavoidable, correct potassium and magnesium and monitor ECG.
