Summary
Profound hypotension and loss of consciousness have been reported when 5-HT3 antagonists are given with apomorphine. Dolasetron also prolongs the QT interval, adding an arrhythmia concern.
What this interaction means in plain words
Apomorphine and Dolasetron are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Apomorphine belongs to Central Nervous System; Dolasetron belongs to Gastrointestinal Tract.
Why it happens
Mechanism of the hypotension is not established; additive QT prolongation is also possible.
What to do
Do not combine. Use a non-5-HT3 antiemetic such as domperidone or trimethobenzamide with apomorphine, according to local availability.
