Summary

Dofetilide has a narrow therapeutic window with a dose-related risk of torsades de pointes, and dolasetron adds further QT, QRS and PR prolongation. The combination may precipitate life-threatening ventricular arrhythmia.

What this interaction means in plain words

Dofetilide and Dolasetron are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dofetilide belongs to Cardiovascular; Dolasetron belongs to Gastrointestinal Tract.

Evidence level: Moderate

Why it happens

Additive prolongation of cardiac repolarization through potassium channel blockade, on top of dofetilide's potent IKr inhibition.

What to do

Avoid the combination and use an antiemetic without QT effects, such as a non-serotonergic agent, in patients receiving dofetilide.
