Summary
Both agents can prolong the QT interval, so combined use raises the risk of torsades de pointes, especially with doxepin at antidepressant doses. Additive sedation and anticholinergic effects may also occur.
What this interaction means in plain words
Amisulpride and Doxepin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amisulpride belongs to Central Nervous System; Doxepin belongs to Dermatology.
Why it happens
Additive QT prolongation through cardiac potassium channel blockade, plus additive CNS depression.
What to do
Obtain a baseline ECG and monitor it after starting or changing doses, and correct potassium and magnesium. Avoid the combination in patients with long QT, bradycardia or other QT-prolonging drugs.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
