Summary

Combining an MAO inhibitor with an SNRI can cause serotonin syndrome and severe CNS toxicity with hyperthermia. Duloxetine labeling contraindicates use with MAO inhibitors, and rasagiline labeling advises against combining with antidepressants.

What this interaction means in plain words

Duloxetine and Rasagiline are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Duloxetine belongs to Central Nervous System; Rasagiline belongs to Central Nervous System.

Evidence level: High

Why it happens

MAO-B inhibition (with reduced selectivity at higher doses) combined with serotonin and norepinephrine reuptake inhibition causes excess synaptic monoamines.

What to do

Do not coadminister. Allow at least 14 days after stopping rasagiline before starting duloxetine, and at least 5 days after stopping duloxetine before starting rasagiline.

What to ask your pharmacist
