Summary
Primidone, through its metabolite phenobarbital, induces intestinal and hepatic drug transporters and enzymes and may lower edoxaban exposure. Reduced anticoagulant activity could raise the risk of stroke or venous thromboembolism.
What this interaction means in plain words
Edoxaban and Primidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Edoxaban belongs to Cardiovascular; Primidone belongs to Central Nervous System.
Why it happens
Induction of P-glycoprotein (and possibly other clearance pathways) lowers edoxaban absorption and increases its elimination.
What to do
Avoid the combination where possible and choose a non-inducing antiepileptic. If it cannot be avoided, monitor closely for thromboembolic events and consider an alternative anticoagulant.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
