Defibrotide has profibrinolytic and antithrombotic activity and may add to the bleeding risk of anticoagulants such as edoxaban. Defibrotide labeling contraindicates concomitant systemic anticoagulants or fibrinolytics. Serious hemorrhage is the concern.
What this interaction means in plain words
Defibrotide and Edoxaban are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Defibrotide belongs to Cardiovascular; Edoxaban belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic effects on hemostasis from factor Xa inhibition and defibrotide's profibrinolytic activity.
What to do
Do not co-administer; hold edoxaban while defibrotide is being given. Resume anticoagulation only after defibrotide therapy is finished and bleeding risk has been reassessed.
