Summary

Ritonavir strongly inhibits CYP3A4 and substantially raises eletriptan concentrations, which may cause serious vasoconstrictive cardiovascular and cerebrovascular events. Inhibition persists for days after the course ends.

What this interaction means in plain words

Eletriptan and Nirmatrelvir / ritonavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Ritonavir-mediated strong CYP3A4 inhibition reduces the metabolism of eletriptan.

What to do

Do not use eletriptan during nirmatrelvir/ritonavir treatment or for at least 72 hours after the last dose. Use a non-CYP3A4-dependent triptan or another migraine therapy during that time.

What to ask your pharmacist
