Summary

Atazanavir is a strong CYP3A4 inhibitor and markedly raises eletriptan exposure, increasing the risk of vasoconstrictive and other triptan adverse effects. Eletriptan labeling contraindicates use within 72 hours of potent CYP3A4 inhibitors, including HIV protease inhibitors.

What this interaction means in plain words

Atazanavir and Eletriptan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Atazanavir belongs to Anti Virals; Eletriptan belongs to Central Nervous System.

Evidence level: High

Why it happens

Strong inhibition of CYP3A4-mediated first-pass and systemic metabolism of eletriptan.

What to do

Do not give eletriptan within at least 72 hours of atazanavir. Use a triptan not metabolized by CYP3A4, such as rizatriptan or sumatriptan, with attention to its own precautions.
