Summary

Erythromycin inhibits CYP3A4 and can markedly raise eliglustat levels, which in turn prolongs the PR, QRS and QT intervals in a concentration-dependent way. Erythromycin itself prolongs the QT interval, adding to the arrhythmia risk.

What this interaction means in plain words

Eliglustat and Erythromycin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Eliglustat belongs to Miscellaneous; Erythromycin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

CYP3A4 inhibition raises eliglustat exposure, combined with additive QT prolongation.

What to do

Avoid in poor or intermediate CYP2D6 metabolizers. In extensive metabolizers, reduce eliglustat to once daily and monitor the ECG, or choose a non-interacting antibiotic.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
