Summary

Cytokine release syndrome from elranatamab can suppress CYP enzyme activity and increase exposure to CYP1A2 substrates such as pirfenidone. This may increase pirfenidone adverse effects like nausea, photosensitivity and liver enzyme elevation, mainly during step-up dosing and CRS.

What this interaction means in plain words

Elranatamab and Pirfenidone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Elranatamab belongs to Oncology; Pirfenidone belongs to Miscellaneous.

Evidence level: Low

Why it happens

Cytokine-mediated suppression of CYP1A2 activity reduces pirfenidone clearance.

What to do

Monitor for pirfenidone toxicity and liver enzymes during elranatamab initiation and any CRS episode. Consider temporary pirfenidone dose reduction if adverse effects emerge.

What to ask your pharmacist
