Summary
Cytokine release syndrome from elranatamab can transiently suppress CYP enzymes, which may increase exposure to CYP1A2 substrates such as ropinirole. This could amplify ropinirole adverse effects like hypotension and somnolence around step-up dosing and during CRS.
What this interaction means in plain words
Elranatamab and Ropinirole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Elranatamab belongs to Oncology; Ropinirole belongs to Central Nervous System.
Why it happens
Cytokine-mediated suppression of CYP1A2 reduces ropinirole clearance during CRS.
What to do
Monitor for ropinirole adverse effects, especially orthostatic hypotension and sedation, during step-up dosing and any CRS episode. Consider a temporary dose reduction if symptomatic.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
