Summary
Cobicistat is a strong CYP3A4 inhibitor and can raise concentrations of temsirolimus and its active metabolite sirolimus. This may increase mTOR-related toxicities such as mucositis, myelosuppression, hyperglycemia, hyperlipidemia and pneumonitis.
What this interaction means in plain words
Elvitegravir / cobicistat / emtricitabine / tenofovir df and Temsirolimus can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Cobicistat inhibits CYP3A4 (and P-gp), reducing metabolism of temsirolimus and sirolimus.
What to do
Avoid the combination if possible and consider an alternative antiretroviral regimen. If unavoidable, reduce the temsirolimus dose and monitor blood counts, glucose, lipids and toxicity closely.
