Summary

Halogenated anesthetics such as isoflurane sensitize the myocardium to catecholamines and indirect sympathomimetics. Ephedrine given during isoflurane anesthesia can provoke ventricular arrhythmias and marked blood pressure swings. The effect is usually less than with halothane but remains relevant.

What this interaction means in plain words

Ephedrine and Isoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ephedrine belongs to Cardiovascular; Isoflurane belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Volatile anesthetics sensitize the myocardium to adrenergic stimulation, and ephedrine releases norepinephrine and directly stimulates adrenergic receptors.

What to do

Titrate ephedrine in small incremental doses under continuous ECG and blood pressure monitoring. Treat arrhythmias promptly and consider an alternative vasopressor if ventricular ectopy develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
