Summary
Sevoflurane sensitizes the myocardium to catecholamines, although less so than halothane. Systemic or high-dose epinephrine may therefore provoke ventricular arrhythmias and hypertension.
What this interaction means in plain words
Epinephrine and Sevoflurane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Epinephrine belongs to Endocrine; Sevoflurane belongs to Central Nervous System.
Why it happens
Volatile anesthetic sensitization of the myocardium to the arrhythmogenic effects of beta-adrenergic stimulation.
What to do
Use the lowest effective epinephrine dose and titrate it carefully, with continuous ECG and blood pressure monitoring. Limit the dose of local infiltration epinephrine and treat arrhythmias promptly.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
