Summary

Risperidone blocks alpha-1 receptors, so epinephrine's vasoconstrictor effect can be reduced and its beta-mediated vasodilation may produce hypotension and tachycardia. Both agents may also have effects on the QT interval, but with inhaled racemic epinephrine systemic exposure is generally low.

What this interaction means in plain words

Epinephrine racemic and Risperidone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Pharmacodynamic opposition at alpha-adrenergic receptors, with possible additive cardiac effects.

What to do

Monitor blood pressure and heart rate if systemic or high-dose epinephrine is needed; avoid epinephrine for treating risperidone-induced hypotension.
