Summary
NSAIDs can blunt the blood pressure lowering effect of eplerenone and, together with it, raise the risk of hyperkalemia and renal impairment. The risk is greater with renal impairment, diabetes or concomitant ACE inhibitor/ARB use.
What this interaction means in plain words
Eplerenone and Ibuprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eplerenone belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins causes sodium retention and reduced renal potassium excretion, opposing and compounding eplerenone's effects.
What to do
Monitor serum potassium, creatinine and blood pressure when starting or stopping the NSAID. Use the lowest ibuprofen dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
