Summary
Adding an angiotensin receptor blocker to an ACE inhibitor produces dual renin-angiotensin blockade, with increased risk of hypotension, hyperkalemia and renal impairment without clear added benefit. The thiazide only partly offsets the potassium effect.
What this interaction means in plain words
Eprosartan and Hydrochlorothiazide-lisinopril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eprosartan belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Dual blockade of the renin-angiotensin-aldosterone system reduces aldosterone and glomerular filtration pressure, causing potassium retention and reduced renal function.
What to do
Avoid combining an ARB with an ACE inhibitor in general. If unavoidable, monitor potassium, creatinine and blood pressure closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
