Summary
Ethacrynic acid is a potent loop diuretic that, added to a thiazide, produces strong sequential nephron blockade with greater fluid, sodium and potassium loss. This raises the risk of hypokalemia, hypomagnesemia, volume depletion, hypotension and renal impairment, especially when combined with an angiotensin receptor blocker.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.
Why it happens
Additive diuresis and renal potassium and magnesium wasting from loop and thiazide diuretics, with volume depletion increasing ARB-related hypotension and renal risk.
What to do
Monitor electrolytes, renal function, blood pressure and volume status after starting or changing doses. Replace potassium and magnesium as needed and reduce diuretic doses if volume depletion occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
