Summary
Adding furosemide to a thiazide-ARB combination produces strong sequential diuresis, which can lead to volume depletion, hypotension, electrolyte disturbances and a decline in renal function. Potassium may fall because of both diuretics, although the ARB offsets this partially.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Furosemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Furosemide belongs to Cardiovascular.
Why it happens
Additive diuretic effects at different nephron sites cause volume and electrolyte depletion, and RAS blockade reduces compensatory glomerular filtration pressure.
What to do
Monitor blood pressure, serum potassium, sodium and creatinine after initiation or dose changes. Reduce the diuretic dose if volume depletion or hypotension develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
