Summary
Piroxicam can diminish the antihypertensive effect of the eprosartan and hydrochlorothiazide combination. Together with an ARB and a diuretic, it increases the risk of acute kidney injury and hyperkalemia, especially in elderly or volume-depleted patients.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Piroxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Piroxicam belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis leads to sodium retention and decreased renal perfusion, counteracting the ARB and diuretic and impairing kidney function.
What to do
Monitor blood pressure, serum creatinine and potassium after starting piroxicam, and maintain hydration. Use the lowest effective dose for the shortest duration or choose a non-NSAID analgesic.
What to ask your pharmacist
