Summary

Ergotamine is a potent CYP3A4 substrate and its toxicity is increased by CYP3A4 inhibitors, but pimozide is not a meaningful inhibitor of this enzyme. Any interaction is largely theoretical, although both drugs are used by patients who may also take other interacting agents. Ergotamine can cause vasoconstriction and has been linked to serotonergic effects.

What this interaction means in plain words

Ergotamine and Pimozide have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Ergotamine belongs to Central Nervous System; Pimozide belongs to Central Nervous System.

Evidence level: Low

Why it happens

No established pharmacokinetic mechanism; theoretical pharmacodynamic overlap.

What to do

No routine change is needed, but check the patient's full list for CYP3A4 inhibitors that would affect ergotamine. Monitor for peripheral ischemia or serotonin toxicity symptoms.

What to ask your pharmacist
