Summary

Both erythromycin and perphenazine can prolong the QT interval, so using them together raises the risk of torsades de pointes and other serious ventricular arrhythmias. Risk is greater with electrolyte disturbance, cardiac disease or other QT-prolonging drugs.

What this interaction means in plain words

Erythromycin and Perphenazine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Erythromycin belongs to Anti Bacterial; Perphenazine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive blockade of cardiac repolarizing potassium currents (hERG) causing QT prolongation; erythromycin may also modestly raise perphenazine exposure through CYP inhibition.

What to do

Prefer a non-macrolide antibiotic such as a non-QT-prolonging alternative. If unavoidable, obtain a baseline ECG, correct potassium and magnesium, and monitor the QTc.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
