Summary
Both drugs can prolong the QT interval, and combined use may add to the risk of torsades de pointes. Risk is greater with higher escitalopram doses, electrolyte disturbances, bradycardia, structural heart disease or hepatic or renal impairment.
What this interaction means in plain words
Escitalopram and Ranolazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Escitalopram belongs to Central Nervous System; Ranolazine belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic QTc prolongation from ranolazine and escitalopram.
What to do
Obtain a baseline ECG and correct potassium and magnesium before combining. Repeat the ECG after dose changes and keep escitalopram at the lowest effective dose.
