Summary
Clarithromycin markedly raises ranolazine concentrations, increasing dose-related QT prolongation, dizziness and nausea. Ranolazine is contraindicated with strong CYP3A inhibitors.
What this interaction means in plain words
Clarithromycin and Ranolazine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Clarithromycin belongs to Anti Bacterial; Ranolazine belongs to Cardiovascular.
Why it happens
Strong CYP3A4 and P-gp inhibition reduces ranolazine metabolism and clearance.
What to do
Do not combine; use azithromycin or another antibiotic, or withhold ranolazine during a short clarithromycin course.
