Summary

Esomeprazole raises gastric pH, which can impair the dissolution and absorption of oral iron salts and iron complexes. Long-term acid suppression may blunt the response to oral iron and contribute to persistent iron deficiency. Parenteral iron dextran is not affected.

What this interaction means in plain words

Esomeprazole and Iron dextran complex have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Reduced gastric acidity decreases solubilization and reduction of ferric iron, lowering oral iron absorption.

What to do

Monitor hemoglobin and ferritin in patients on long-term PPI and oral iron, and consider a higher iron dose or switching to intravenous iron if response is inadequate.
