Summary
Esomeprazole inhibits CYP2C19 and can raise mavacamten exposure. Excess mavacamten can reduce left ventricular ejection fraction and cause heart failure from systolic dysfunction.
What this interaction means in plain words
Esomeprazole and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Esomeprazole belongs to Gastrointestinal Tract; Mavacamten belongs to Cardiovascular.
Why it happens
Inhibition of CYP2C19-mediated metabolism of mavacamten increases its plasma concentrations.
What to do
Avoid the combination; use an acid suppressant without CYP2C19 inhibition (e.g., an H2 blocker, or per labeling) if required. If started or stopped, follow label guidance with echocardiographic monitoring of ejection fraction.
