Drug interactions
clopidogrel + esomeprazole
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Esomeprazole inhibits CYP2C19, decreasing clopidogrel’s active metabolite and antiplatelet effect; may increase risk of thrombotic events. Concomitant use is not recommended.
Mechanism of action
CYP2C19 inhibition by esomeprazole reduces biotransformation of clopidogrel to its active thiol metabolite → diminished platelet inhibition.
Clinical management
Avoid this combination. If acid suppression is needed, use a PPI with minimal CYP2C19 inhibition (e.g., pantoprazole) or an H2 blocker. Staggering doses does not mitigate. Monitor for ischemic/cardiovascular events.
