Summary

Glecaprevir and pibrentasvir inhibit intestinal P-glycoprotein and may modestly raise exposure to estradiol. Clinically, this is expected to cause little change, though estrogenic adverse effects could occur.

What this interaction means in plain words

Estradiol and Glecaprevir / pibrentasvir have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Inhibition of P-glycoprotein efflux (and possibly BCRP) increases estradiol absorption.

What to do

No routine dose change is needed; watch for estrogenic effects such as breast tenderness or nausea and reduce the estradiol dose if they occur. Note that ethinylestradiol-containing products are a separate concern with this HCV regimen because of ALT elevations.

What to ask your pharmacist
