Summary
Live vaccinia vaccine can cause progressive vaccinia and other serious complications in immunosuppressed patients. TNF blockade increases this risk. Vaccine response may also be unreliable.
What this interaction means in plain words
Etanercept and Smallpox vaccine live are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
TNF inhibition impairs immune containment of replicating vaccinia virus.
What to do
Avoid live vaccinia vaccine in patients on etanercept unless the risk of exposure clearly outweighs the risk. Consider a non-replicating vaccine alternative where available.
