Summary

Ethacrynic acid is a loop diuretic that causes potassium loss, and salmeterol can further lower serum potassium. Together they may produce clinically significant hypokalemia, which increases arrhythmia risk, particularly in patients with heart disease or on digoxin.

What this interaction means in plain words

Ethacrynic acid and Fluticasone-salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Ethacrynic acid belongs to Cardiovascular; Fluticasone-salmeterol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive hypokalemia from renal potassium wasting (loop diuretic) and beta2-mediated cellular potassium uptake.

What to do

Monitor serum potassium and magnesium and replace as needed. Take extra care in patients with COPD exacerbations, hypoxia or cardiac disease.
