Summary
Combining a loop diuretic with a thiazide produces strong sequential nephron blockade. This can cause marked diuresis, volume depletion, hypokalemia, hyponatremia, hypotension and renal impairment. Ethacrynic acid also adds ototoxicity risk at high doses.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide-telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide-telmisartan belongs to Cardiovascular.
Why it happens
Additive diuresis from blockade of the loop Na-K-2Cl cotransporter and the distal sodium-chloride cotransporter.
What to do
Monitor electrolytes, renal function, weight and blood pressure, and reduce diuretic doses if volume depletion occurs. Replace potassium and magnesium as needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
