Summary
Combining a loop diuretic with hydrochlorothiazide and valsartan produces strong diuresis with a risk of hypokalemia, hyponatremia, volume depletion, hypotension and acute kidney injury. Ethacrynic acid also carries a risk of ototoxicity, especially at high doses or with renal impairment.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Sequential nephron blockade by loop and thiazide diuretics gives additive natriuresis and potassium loss, with ARB-related reduction in glomerular filtration pressure.
What to do
Monitor electrolytes, renal function, weight and blood pressure, particularly after starting or increasing doses. Adjust diuretic doses and replace potassium or magnesium as needed.
What to ask your pharmacist
