Summary
Drospirenone has anti-mineralocorticoid activity and can raise serum potassium. Systemic beta-blockade with timolol can modestly impair cellular potassium uptake, so additive hyperkalemia is possible, mainly in patients with renal impairment or other risk factors.
What this interaction means in plain words
Ethinyl estradiol-drospirenone and Timolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Ethinyl estradiol-drospirenone belongs to Endocrine; Timolol belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic effect: aldosterone antagonism by drospirenone plus reduced beta-2-mediated cellular potassium uptake from beta-blockade.
What to do
Monitor serum potassium in patients with renal impairment, diabetes or other hyperkalemia risks, particularly in the first cycle of combined use. Ophthalmic timolol carries less risk.
