Summary

Stiripentol has mixed inhibitory and inducing effects on CYP3A4, and etoposide is partly cleared by this enzyme. Etoposide exposure may therefore rise or fall unpredictably, affecting either toxicity (myelosuppression) or antitumour efficacy. The size of the effect is not well characterised.

What this interaction means in plain words

Etoposide and Stiripentol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Etoposide belongs to Oncology; Stiripentol belongs to Central Nervous System.

Evidence level: Low

Why it happens

Stiripentol both inhibits and induces CYP3A4, altering the metabolic clearance of etoposide in an unpredictable direction.

What to do

Monitor blood counts and clinical response to etoposide when stiripentol is started or stopped. Adjust the etoposide dose if marked toxicity or reduced effect is observed.

What to ask your pharmacist
