Summary

mTOR inhibitors such as everolimus raise the risk of angioedema in patients taking ACE inhibitors. Swelling of the face, lips, tongue or airway may occur and can be life-threatening.

What this interaction means in plain words

Everolimus and Perindopril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Everolimus belongs to Oncology; Perindopril belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Both impair bradykinin breakdown or increase its effects, with mTOR inhibition adding to ACE inhibitor-related bradykinin accumulation.

What to do

Prefer a non-ACE-inhibitor antihypertensive where possible. If combined, counsel patients to seek urgent care for swelling and stop perindopril if angioedema occurs.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
