Summary
Tacrolimus can prolong the QT interval, and ezogabine has a small additional QT effect. The combination may modestly increase the risk of ventricular arrhythmias, especially with hypokalemia or hypomagnesemia, which tacrolimus can itself cause.
What this interaction means in plain words
Ezogabine and Tacrolimus can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ezogabine belongs to Central Nervous System; Tacrolimus belongs to Miscellaneous.
Why it happens
Additive pharmacodynamic prolongation of cardiac repolarization, with tacrolimus-related electrolyte disturbances adding to the risk.
What to do
Check a baseline ECG and monitor the QTc, potassium and magnesium during co-therapy. Replace electrolytes as needed and reassess the combination if the QTc lengthens markedly.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
