Summary
Both drugs can prolong the QT interval, and fingolimod also causes bradycardia, which is a risk factor for torsades de pointes. Combined use may raise the likelihood of a serious ventricular arrhythmia, particularly at treatment initiation.
What this interaction means in plain words
Fingolimod and Gemifloxacin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fingolimod belongs to Central Nervous System; Gemifloxacin belongs to Anti Bacterial.
Why it happens
Additive QT prolongation (fluoroquinolone hERG blockade plus fingolimod-related bradycardia and QT effects).
What to do
Avoid the combination where an alternative antibiotic exists. If it is unavoidable, obtain a baseline ECG, correct potassium and magnesium, and monitor ECG and heart rate.
What to ask your pharmacist
