Summary
Fingolimod slows heart rate when started, and beta-blockers add to this effect, increasing the risk of significant bradycardia and AV block. The risk is greatest in the hours after the first dose or after a treatment interruption.
What this interaction means in plain words
Fingolimod and Labetalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fingolimod belongs to Central Nervous System; Labetalol belongs to Cardiovascular.
Why it happens
Additive negative chronotropic effects: fingolimod activates cardiac sphingosine-1-phosphate receptors while labetalol blocks beta receptors.
What to do
Obtain an ECG before starting fingolimod and monitor heart rate and blood pressure for at least 6 hours after the first dose. Consider an alternative to the beta-blocker if feasible, or extend monitoring.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
