Summary
Both drugs can prolong the QT interval and slow cardiac conduction. Combined use raises the risk of ventricular arrhythmias including torsades de pointes, especially with electrolyte disturbances or structural heart disease. Ziprasidone is generally avoided with other QT-prolonging antiarrhythmics.
What this interaction means in plain words
Flecainide and Ziprasidone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Flecainide belongs to Cardiovascular; Ziprasidone belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic effects on cardiac repolarization and conduction.
What to do
Avoid the combination where possible and choose an alternative antipsychotic with lower QT liability. If unavoidable, obtain a baseline and follow-up ECG and correct potassium and magnesium.
What to ask your pharmacist
