Summary

Gemtuzumab ozogamicin carries a labeled warning regarding QT prolongation and fluconazole can also prolong the QT interval. Together they may increase the risk of ventricular arrhythmias including torsades de pointes, especially with electrolyte disturbances, which are common in patients with leukemia.

What this interaction means in plain words

Fluconazole and Gemtuzumab can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive prolongation of cardiac repolarization (QTc interval).

What to do

Check an ECG and electrolytes (potassium, magnesium, calcium) before treatment and monitor periodically, correcting abnormalities. Consider an alternative antifungal with less QT effect if the QTc becomes prolonged.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
