Summary

Fluconazole can raise dihydroergotamine concentrations by inhibiting CYP3A4. Higher exposure raises the risk of ergotism with severe peripheral and cerebral vasospasm and ischemia.

What this interaction means in plain words

Dihydroergotamine and Fluconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dihydroergotamine belongs to Central Nervous System; Fluconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated dihydroergotamine metabolism by fluconazole.

What to do

Avoid the combination and choose another migraine therapy, such as a triptan, while fluconazole is being used. Ensure that patients know the early signs of ischemia.
