Summary
Co-administration is contraindicated. Fluconazole raises domperidone plasma levels, and both drugs prolong the QT interval. Together they increase the risk of QT prolongation, torsades de pointes, serious ventricular arrhythmias, and sudden cardiac death.
What this interaction means in plain words
Domperidone and Fluconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Domperidone belongs to Gastrointestinal Tract; Fluconazole belongs to Anti Fungals.
Why it happens
Pharmacokinetic: Fluconazole is a moderate-to-strong CYP3A4 inhibitor (dose-dependent). Domperidone is extensively metabolized by CYP3A4, so fluconazole reduces its clearance and increases exposure. Pharmacodynamic: Domperidone blocks hERG channels and prolongs QTc, and fluconazole independently prolongs QTc. The effects on cardiac repolarization are additive.
What to do
Avoid the combination. If antifungal therapy is required, stop domperidone.
